What is lipedema?

Lipedema is a pathological change in adipose tissue which, as it progresses, can lead to a considerable reduction in the quality of life. Usually only women are affected by lipedema.

The starting point of the disease is usually an initial hormonal change, such as the onset of puberty, pregnancy or - rarely - menopause: during these hormonal upheaval phases in women, there is a surge of fatty tissue in typical areas of the disease: thighs, lower legs, inside of the knees , Hips, buttocks and also arms.

The affected fat cells appear to be pumped up due to the accumulation of lymphatic fluid associated with the disease. In advanced lipedema, e.g. in stage 3, grotesque body shapes with overhanging skin-fat flaps can arise, which are then also associated with considerable pain in addition to the restricted movement.

How do you recognize lipedema?

An increase in the circumference of the legs and arms beginning with puberty, pregnancy or menopause is suspect. The skin is irregular on its surface and very sensitive to touch. Hands and feet are still slim. Exercise and a healthy diet have no significant effect on the development of the body’s silhouette. Blue spots appear quickly, even with the slightest trauma. Another person in the family is also affected by this appearance.

All patients who come to our specialist clinic in order to receive the best possible treatment should have received the specialist diagnosis “lipedema” from a phlebologist, vascular surgeon or another specialist familiar with this clinical picture before the first consultation appointment. Ideally, you should also be provided with flat-knit compression garments and wear them consistently.

By making a diagnosis with a specialist, other causes of weight gain such as venous insufficiency with the help of sonography and other clinical examination methods are excluded. If you have varicose veins accompanying your lipedema, vein stripping or some other type of obliteration of the varicose veins should be performed before lipedema surgery.

How are lipedema divided?

From the experienced surgeon in the Park-Klinik Birkenwerder near Berlin, liposuction is a very low-risk procedure, however, it is not possible to exclude developing skin retractions or contour irregularities (dents).

3 stages are divided:

  1. Stage: Formation of orange peel (minor dents)
  2. Stage: Development of larger dents (mattress skin)
  3. Stage: large, harder flaps of skin and ridges, which even cover the hands and back of the feet

In addition, depending on the severity, there are 5 types:

  1. Type: limited to the buttock-hip region
  2. Type: up to the knees and fat rag on the knee inside
  3. Type: to the ankles
  4. Type: only hands and feet are not affected
  5. Type: Water retention (edema) even in the hands and back of the feet, fingers and toes (lipolymph edema), damage to the larger lymphatics

The partial bruising that occurs during liposuction disappears after a few weeks.

What does this therapy look like?

The gentle Waterjet Assisted Liposuction (WAL) removes the diseased lipoedema fat cells without destroying the connective tissue structures, blood vessels, lymph channels and nerves. This can be aesthetically quite beautiful results on the affected body regions.

On the day of the operation, the surgeon first marks the operating area with a pen. About a few tiny small punctures then a saline solution is injected. It also contains adrenaline and a local anesthetic. The solution quenches the bleeding that occurs during aspiration, soothes the pain of the patient and causes the fat cells stored there to swell, making them easier to remove. Using a small cannula, you can then aspirate up to 9 liters of fat cells, lymph and blood from the subcutaneous fatty tissue.

The operations take about 120 minutes each. An anesthetist is always present accompanying the operation and can offer either a drowsy sleep or a general anesthetic if necessary. As a rule, one night is planned in the clinic. For further safety and avoidance of complications, maximum fat removal of up to 10% of body weight should not be exceeded. Also, the operated body surface must be kept as low as possible, which is why lipoedema must always be operated in several steps.

What is WAL (water jet associated liposuction) and how does such an operation work?

On the day of the operation, the surgeon first marks the operation area with a pen. A saline solution is then injected into the fatty tissue through several tiny punctures. The solution also contains adrenaline and a local anesthetic. The solution stops the bleeding that occurs during suction, relieves the patient's pain and allows the fat cells stored there to swell so that they can be removed more easily. With the help of a narrow cannula, up to 9 liters of fat cells, lymph and blood are sucked out of the subcutaneous fatty tissue.

The technique of the water jet gently treats the lymphatic system. These are of particular importance for regeneration and the decongestion process. Thanks to the power of the water jet, we get a maximum amount of extracted fat with very attractive aesthetic results at the same time.

The operation takes about 120 minutes each. An anesthetist is always present during the operation and can offer either twilight sleep or general anesthesia if necessary. As a rule, an overnight stay in the clinic is planned. There is an upper limit to the volume to be aspirated. This is determined in a personal consultation.

What is LASER tightening?

Since January 2022 we have been offering our patients an optional LASER tightening treatment (in addition to water jet-associated liposuction). With this method, the fat cells are specifically destroyed with controlled and precise laser energy and liquefied by the heat (fat melt). The liquefied fat - depending on the size of the area to be treated - is broken down by the metabolism or sucked off manually.

The thermal effect of the laser also stimulates the formation of the skin's own collagen. In this way, LASER tightening treatment not only reduces fat, but also tightens the subcutaneous connective tissue.

Will life be changed for the better after lipedema surgery?

Lipedema surgery can change your life positively by improving lymphatic drainage. This reduces the volume of the extremities and makes the leg or arm slim again. This leads to the fact that the mobility increases again, sport is again possible without restrictions. In addition, pain can be reduced or even eliminated entirely. Items of clothing such as pants, blouses or tight boots can be worn again and secondary diseases such as joint problems are avoided.

Ich habe mich in der Klinik wegen meines Lipödems (Stadium 2, Typ 3–4) zweimal operieren lassen im Februar an den Vorderseiten und im Mai an den Hinterseiten der Beine. Zwar wurde mein erster OP-Termin kurzfristig abgesagt, da kein Nachtpersonal verfügbar war, aber Frau Jastak setzte sich sofort persönlich ein und ermöglichte mir innerhalb einer Woche einen Ersatztermin. Das…Ich habe mich in der Klinik wegen meines Lipödems (Stadium 2, Typ 3–4) zweimal operieren lassen im Februar an den Vorderseiten und im Mai an den Hinterseiten der Beine. Das fand ich großartig und zeigt den Einsatz für die Patientinnen.

Die Operationen verliefen sehr gut. Nach der zweiten OP hatte ich starke Wadenschmerzen und konnte kaum laufen. Die Klinik reagierte sofort, konnte jedoch nichts feststellen und schickte mich zur Sicherheit in die Notaufnahme, um eine Thrombose auszuschließen. Auch wenn die Ursache unklar blieb, fühlte ich mich stets ernst genommen und gut betreut. Nach zwei Wochen waren die Beschwerden fast verschwunden.

Mit dem Ergebnis an den Vorderseiten bin ich sehr zufrieden, an den Hinterseiten dauert die Heilung wegen Schwellungen und Wassereinlagerungen noch an. Bei der Nachkontrolle wurde mir das ausführlich erklärt, und ich habe schon einen Folgetermin im November die Nachsorge wird hier also sehr ernst genommen.

Insgesamt fühle ich mich in der Klinik medizinisch wie menschlich bestens aufgehoben. Besonders danke ich Frau Jastak für ihr großes Engagement und dem ganzen Team für die tolle Betreuung!

Treatment: Lipödem-Behandlung
Meine erst Liposuktion des Lipödems Stadium 2 war 09.05.2019 in der Park-Klinik Birkenwerder. Lange habe ich überlegt diesen Eingriff überhaupt machen zu lassen. Den entscheidenden Ausschlag gab ein Erfahrungsaustausch mit Frauen, die dieselbe Diagnose hatten und sich daraufhin operieren ließen. Schmerzfreiheit und eine viel höhere Lebensqualität waren das durchweg positive…Meine erst Liposuktion des Lipödems Stadium 2 war 09.05.2019 in der Park-Klinik Birkenwerder. Lange habe ich überlegt diesen Eingriff überhaupt machen zu lassen. Den entscheidenden Ausschlag gab ein Erfahrungsaustausch mit Frauen, die dieselbe Diagnose hatten und sich daraufhin operieren ließen. Schmerzfreiheit und eine viel höhere Lebensqualität waren das durchweg positive Ergebnis. Daraufhin war es für mich nur noch eine Frage des „Wo“ und nicht mehr des „Ob“ einer OP.

Da ich recht weit entfernt wohne, nahm ich die Möglichkeit in Anspruch, das Beratungsgespräch telefonisch durchzuführen. Die Dame am Telefon – Frau Azdasht – war sehr nett. Sie klärte mich über den Eingriff auf und beantwortete mir anschließend alle noch offenen Fragen. Einen Tag vor der OP hatte ich mein Vorbereitungsgespräch mit dem Operateur und dem Anästhesieteam. Es verlief kurz und unkompliziert. Es wurde die Standartfragen geklärt (Allergien etc.) und welcher Bereich zuerst operiert werden sollte. Anschließend wurden Vorherbilder gemacht und die Masse für das OP-Mieder genommen.

Meine OP war für den Folgetag 14 Uhr angesetzt. Die Vorbereitung lief völlig problemlos: Zimmer beziehen, für den OP aus-/umziehen. Dann ging es schon in den OP-Bereich. Ich war nicht sonderlich nervös. Was vermutlich daran lag, dass ich mich sehr gut aufgehoben fühlte. Nachdem die Anästhesieärztin den Zugang sehr gut gestochen hatte, zeichnete der Operateur die zu operierenden Stellen an und es ging in den OP. Dort wurden meine Beine mit zwei verschiedenen Lösungen eingerieben, bevor ich mich auf den OP Tisch legte. An mehr kann ich mich nicht erinnern, da der Dämmerschlaf bei mir leider nicht optimal funktionierte und ich aus diesem Grund doch in Vollnarkose versetzt wurde. Nach der OP war meine erste Frage, wieviel Liter entnommen werden konnten… „4 Liter aus den Innenseiten der Oberschenkel!“ Klasse! Ich war schon wieder auf dem Zimmer, als ich diese Frage stellen konnte. Anfangs war mir noch etwas dämmrig zu Mute, doch dann war ich recht schnell wieder voll da. Beim Wachwerden verspürte ich ein starkes Brennen, welches aber nicht lange anhielt. Schmerzmittel brauchte ich weder am Abend noch in der Nacht. Recht schnell konnte ich das Bett verlassen, um das Laufen zu probieren. Mein Kreislauf war fit und der erste Versuch funktionierte bereits sehr gut. Das Ergebnis der OP konnte ich natürlich nur durch das Mieder erahnen. Trotzdem konnte ich unter diesen Umständen bereits jetzt einen großen Unterschied zwischen „vor“ und „nach“ der OP feststellen. Die anschließende Nacht war gut und morgens durfte ich auch schon duschen. Meine Einschnitte nässten noch bluteten überhaupt nicht. Am nächsten Morgen nach der Visite konnte ich die Heimreise antreten. Mein Freund holte mich ab. Die Fahrt war lang und die Beine begannen schon während der Fahr zu schmerzen. Daher nahm ich -zuhause angekommen- meine erste (von insgesamt fünf) Schmerztablette.

Die Heilung verlief super. Die heftigen Blutergüsse und Schwellungen ließen nach vierzehn Tagen (in der Zeit war ich auch krankgeschrieben) nach. Eine Mischung aus „viel laufen“ und „Beine hoch legen“ trug wesentlich zur schnellen Besserung bei und tat mir sehr gut. Die erste halbe Stunde laufen war oft schmerzhaft, danach ging es gut. Nach ca. 2,5-3 Wochen waren die Blutergüsse komplett weg und die Schwellung war auch sehr gut zurückgegangen. Verhärtungen waren nach ca. 12-14 Wochen so gut wie weg. Nach der ersten OP konnte ich schon eine große Veränderung feststellen: Treppensteigen und Wandern fiel mir schon deutlich leichter. Mein Gangbild hat sich auch verbessert.

2. OP
Meine zweite Liposuktion (bei Lipödem) fand Ende Dezember 2019 statt (OS Vorderseite und Außenseite). Der Ablauf entsprach dem der ersten OP. Die OP verlief gut und innerhalb des Zeitplans. Mein Kreislauf blieb auch im Anschluss konstant. Nur aus den Schnittwunden trat am ersten Tag noch Wundflüssigkeit aus. Bei der ersten OP blieb dies aus. Ich denke das hat zur noch schnelleren Heilung beigetragen. Das Taubheitsgefühl an den operierten Stellen hielt ungefähr 8 Wochen. Nichts desto trotz konnte ich die ersten Schritte schon am Tag der OP machen und schnell aufrecht und flott gehen. Schmerzmittel habe ich nur am Tag nach der OP genommen, um die lange Heimfahrt und das Wechseln der Kompressen ohne Schmerzen zu bewerkstelligen. Während meinem Aufenthalt in der Klinik habe ich mich wieder sehr gut aufgehoben und kompetent beraten und betreut gefühlt. Dafür möchte ich mich ausdrücklich beim Pflegeteam und den Ärzten der Park-Klinik Birkenwerder bedanken. Im Ergebnis kann ich festhalten, dass die Muskeln an den Oberschenkeln vorne jetzt sehr gut sichtbar sind. Mein Gangbild ist besser (die Beine stehen enger zusammen), die Rückenschmerzen sind besser bzw. weg, mein Stoffwechsel funktioniert, meine Beine fühlen sich leichter an und ich habe mehr Kondition. Der Unterschied zur Ausgangssituation ist wirklich gravierend.

3. OP
Meine dritte OP war am 27.04.2020. Was soll ich sagen: Es lief wieder alles super. Der Ablauf bei Klinikteam ist eingespielt. Sogar in Zeiten von Corona. Alle sehr nett und freundlich/lustig. Auch bei dieser OP war ich nicht aufgeregt. Ich fühle mich eben einfach in guten Händen. Die bisherige Heilung ist noch besser verlaufen als zuvor. Die Blutergüsse sind nach 14 Tagen so gut wie raus gewesen und sitzen klappt auf meinem Gymnastikball sehr gut. Meine Physio ist regelrecht begeistert vom Ergebnis. Optisch und grade vom Heilungsprozess. Verhärtungen sind da, aber weniger als die male zuvor. Seit dem 11.05.2020 bin ich wieder arbeiten, was auch ohne Probleme sehr gut klappt. Das Nachher-Bild ist genau 3 Wochen nach der OP entstanden. Es wurde die OS Rückseite und der Po gemacht. Gesamt sind jetzt aus den Oberschenkeln und Po 12,5 Liter Lipfett entfernt worden. Wenn ich diese Zahl lese oder ausspreche wird mir klar was ich an Masse bewegen musste. Ca. 1,5 Wochen nach der OP war ich mit einer Freundin wandern. Als es den Hiwwel (Berg) raufging, haben mir nicht etwa die operierten Stellen wehgetan, NEIN es waren die Waden. Diese sind noch nicht operiert und ich spüre den Unterschied ganz genau, so dass ich schon jetzt mit einer freudigen Erwartung auf die nächste OP schaue.
Mein Fazit lautet daher: Macht die OP und macht sie in der Parkklink Birkenwerder. Eine absolut tolle Klinik in jeder Hinsicht!
